*Our office provides Sports Participation Exams for those who need documentation for school athletic teams and have had a full Physical Exam (Well Child Check) with us within the past year, but outside the sports team's required timeframe. This service is not covered by insurance and is offered for a flat fee (self pay) of $75.00. The visit will focus solely on the required exam components and paperwork for sports participation. Any additional medical concerns must be addressed in a separate visit and will be billed to your insurance. |
