At Concord Pediatrics, we always welcome new patients! Here are a few things to know about our practice that set us apart from other pediatrician offices.
Concord Pediatrics, P.A. was established in 1961 and for over 50 years our mission has been to provide each patient with the best possible pediatric care, personalized to each patient. We are privately owned and are committed to providing convenient access to our family for not only preventative care, but same day illness related visits as well.
We believe in continuity of care and building strong relationships with our families. We will help you in choosing your child(ren)’s primary care provider and will schedule your preventative care appointments with the primary care provider. For families with more than one child, we can group appointments back-to-back whenever possible for your convenience. On occasion, it may be necessary to see an alternate provider when coming in for visits in between regularly scheduled exams, but whenever possible we will schedule you with your primary care provider.
Our preventative care includes guidance on growth and development including adhering to the immunization guidelines recommended by the American Academy of Pediatrics (AAP). You can find this schedule displayed in our office as well as on our website. We are happy to help you navigate vaccination schedule modifications, but require that children are adequately immunized by each age on the recommended guidelines.
How to Enroll at Concord Pediatrics
For first time parents:
We recommend parents contact our office in advance of giving birth to confirm we are in-network with your insurance and so we can provide you with information about our practice, enrollment forms to fill out before you have your child(ren) and sign you up for our patient portal.
Transferring care for your child or children:
We recommend that you contact the office so we can collect information including medical records from your current provider prior to the first visit. Dates of immunizations are particularly important, as is information about chronic illnesses and medications. Complete records will help our providers give your child the best, most comprehensive medical care at your first visit. We will sign you up for our patient portal and send you enrollment forms. We request that you return all enrollment forms in advance of the first appointment so that we have all the necessary information to treat your child. A pleasant, comfortable first visit builds trust and helps put the child at ease during future visits. We want your child to enjoy getting to know our doctors and staff, so we work hard to establish this bond during every appointment to our office. During your initial appointment, we will explain our office procedures in detail and answer any questions you may have.
Please download and print the forms from this website, fill them out ahead of time and send them to us via your patient portal, or via US Mail.
All forms can be obtained on our Patient Forms page .
